This questionnaire is required if you are making self-employment income
Employment Expenses Questionnaire
This questionnaire is required if you have employment expenses & your employer provided you with the T2200(& TP-64.3 for Québec)
Rental Property Income Questionnaire
This questionnaire is required if you own a property that you are renting out
Medical Expenses Questionnaire
This questionnaire is required if you have several medical expenses to declare
Uber DoorDash Questionnaire
This questionnaire is required if you are generating income from Uber, DoorDash or similar.
RL31 Questionnaire
This questionnaire is required if you rented your property in Quebec for which rent was paid on December 31st
Disposition of Property Questionnaire
This questionnaire is required if you have disposed of (sold) your property within the taxation year
Moving Expenses Questionnaire
This questionnaire is required if you moved at least 40 Km to be closer to your NEW workplace or school
